D6722. Resin over partly precious metal. Some plans call this an optional service or an alternate benefit. The plan pays what the standard version costs. You pay the difference.

A bridge replaces a missing tooth with a false tooth held by crowns on the teeth on either side. It is cemented in and does not come out. Each part is coded separately: the false tooth (pontic) and each anchor crown. Codes differ by material. A rarer design anchors the bridge on inlays or onlays set into the neighboring teeth instead of full crowns.

The teeth on either side of the gap are trimmed for crowns, an impression or scan is taken, and the lab makes the false tooth and its anchors as one piece that is cemented in. It does not come out for cleaning, so a floss threader or a small brush goes underneath it. Every unit is billed on its own line, which is why one missing tooth can appear as three. Most plans file this as Major. What they actually pay varies by plan.
